If there is no ConnectionRequestURL, ConnectionRequestUsername and ConnectionRequestPassword, you will not be able to summon the device from the ACS.

You may have to contact the device vendor.

Dovetailing into what @dustain said.

I’ve found I get much faster results from the CPE vendor by pointing to the exact line of the spec they are violating. In your case, InternetGatewayDevice.ManagementServer.ConnectionRequestUsername. If your vendor purports they conform to v1.0 of the spec, they are missing some required parameters.
